The Science Behind Microneedling How It Works and Its Benefits

An in-depth look at the microneedling process and its benefits for skin rejuvenation

The first thing to know about Microneedling is that you have options, so do your research. The science continues to evolve – and rapidly – with your options only increasing. But let’s start at the beginning when the science was new.

The very first modern Microneedling option came on the scene in 1995 when Dr. Desmond Fernandes utilized hypodermic needles to treat wrinkles and scars. He created what was called a "needle stamp" device to trigger collagen production. The concept itself can be traced all the way back to 1905 when a dermatologist in Germany employed motorized dental burs to treat birthmarks and scars.

      We’ve come a long way since 1905.

The Advance of Science in Microneedling

Today, Microneedling utilizes a handheld device – like a SkinPen, for example - with tiny needles to create controlled injuries in the skin. Those micro-injuries stimulate your skin's natural healing process, which naturally induces the production of collagen and elastin to repair the injuries. This can improve the appearance of your skin by reducing fine lines and wrinkles, large pores, scarring, uneven skin tone, and so much more.

We’ll refer you to one of many PubMed articles that gives much more of a scientific explanation: In part, the article explains:

Micropunctures are created using microneedles which produce a controlled skin injury without actually damaging the epidermis. These microinjuries lead to minimal superficial bleeding and set up a wound healing cascade with release of various growth factors such as platelet derived growth factor (PGF), transforming growth factor alpha and beta (TGF-α and TGF-β), connective tissue activating protein, connective tissue growth factor, and fibroblast growth factor (FGF).[7] The needles also breakdown the old hardened scar strands and allow it to revascularize. Neovascularization and neocollagenesis is initiated by migration and proliferation of fibroblasts and laying down of intercellular matrix.[8,9] A fibronectin matrix forms after 5 days of injury that determines the deposition of collagen resulting in skin tightening persisting for 5–7 years in the form of collagen III.

We’ll stop there and let you read more if you choose. Suffice it to say that Microneedling, at its core, is a way to remodel the skin. The skin becomes thicker, firmer, and smoother – and your skin will look younger and healthier.

      Benefits of Microneedling

Microneedling, depending on the method you choose, can usher in an array of benefits, including:

      • Improving fine lines and wrinkles
      • Reducing scarring, including acne scars and stretch marks
      • Evening out uneven skin tone
      • Shrinking enlarged pores
      • Thickening the skin which thins with age
      • Addressing loose or crepey skin
      • Mild scars, especially those from acne or burns.

The best part is that it’s safe for all skin types and skin colors. It can be used anywhere on your body – including your legs, back and neck. It can also be used year round.

It doesn’t carry the risk of skin discoloration, unlike some other skin rejuvenation procedures, making it ideal for people with darker skin tones.

The procedure can also help people who have medical conditions such as: Alopecia areata (hair loss due to an autoimmune disease) or even Hyperhidrosis (excessive sweating). In some cases, healthcare providers use microneedling to deliver drugs or vaccines directly into your skin.

      Understanding All the Microneedling Options

The benefits you experience can be greatly enhanced using Microneedling combined with other technologies or with certain added substances like PRF. You’ve probably heard of the Vampire Facial, using PRP in conjunction with Microneedling.

It also matters which device you use – and the right one is the right one for you.

The SkinPen was the first FDA-approved Microneedling device. We can use this device with substances that enhance the effects – like platelet-rich fibrin or CBD. We can also use it together with other procedures. Peels, for example, combine well with Microneedling to address skin discoloration conditions like Melasma.

Since then, we’ve also added one of Elle Magazine’s Future of Beauty winners: the Vivace Ultra device. It offers a minimally invasive skin rejuvenation treatment using a combination of microneedling and radiofrequency energy to ramp up collagen and elastin production. Vivace Ultra works not only in the superficial layers of the skin but also in deeper dermal layers, with increased production of collagen and elastin continuing for up to six months – and results that can last years

      Seeing Results

Results vary according to your age, skin type, and how well the skin responds to treatment – and what Microneedling option you choose. Some people may need multiple treatments to achieve their desired results, some people see incredible results after their first treatment. Individual differences in how people respond are hard to predict. Generally, most people need multiple treatments, usually about 3-6 weeks apart. It can take 6 months to see optimal results. It’s advised to repeat the procedure at least once a year for maintenance.

      Is Microneedling Safe for Everyone?

Most people tolerate Microneedling exceptionally well. It’s been around awhile and has an excellent safety profile. But as safe, effective and life changing as Microneedling can be, it’s not for everyone. It may not be a good choice for you if you:

      • Are taking certain medication for acne
      • Are diagnosed with certain blood disorders
      • Are allergic to stainless steel
      • Have moles, freckles, skin tags or other skin growths that bleed or have changed in shape or size
      • Are taking anticoagulants (blood-thinning drugs to prevent blood clots) or any medications, herbs or supplements that can thin blood excessively
      • Have cancer, especially if you are undergoing chemotherapy or radiation
      • Have frequent skin rashes (contact dermatitis)
      • Experience cold sores
      • Have keloidal tendencies.
      • Suffer with skin conditions like psoriasis or eczema

      What to Expect from Microneedling

Before your first Microneedling session, we will carefully examine your skin, take a medical history and discuss your goals. We will also take photos of the area so you can compare your skin before and after Microneedling. This is a good time to ask any questions you may have about your options, risks or side effects, concerns about pain or downtime, or how many sessions you may need.

We thoroughly cleanse the treatment area and apply a numbing gel about 30 minutes to 45 minutes before beginning Microneedling, giving it time to take effect. Needle lengths can range from 0.5 – 3 millimeters. Longer needles go deeper into your skin, which may be needed if you have deep scars.

You may feel warmth or experience a scratching sensation, but usually people only experience discomfort near or on bony areas, like cheekbones. Deep microneedling might cause a little bleeding, but it’s not a common experience.

A session can take anywhere from 15 minutes to an hour, depending on the size of the area being treated and what treatment option you choose.

Prior to beginning, during or after treatment, it may be helpful to use certain skin products. We can discuss this in more detail.

Because Microneedling is an outpatient procedure, you can return home and immediately resume your normal activities. Most people are able to apply makeup the day after the procedure, but you should avoid the sun until your skin heals. Your skin may be red or swollen for up to three days. An ice pack can help reduce swelling, irritation or discomfort.

After Microneedling, you may notice that your skin burns or feels tight. Some people also have dry, flaking skin that improves over time, especially with moisturizer. Depending on the treatment option you select, we will discuss more specifics in detail with you at your first session.
